Database issues on a few files

couple problems I see:

There seams to be a bug where code that should be near the top of the page is being added in the body on the checkout page.

this code block which should be before the chedckout for div tag:

is being added after the checkout form, you will need to manually move that code for the time being.

2) You have the cart and connection using the same name "Summit" you should use unique names for the cart and connection

3) it looks like you already have a orders and order details table in your database that have columns that don't match the presets, try deleting those columns and have the wizard create them for you.

you will need to delete the existing checkout pages, change the name of the connection and delete the orders and order details tables, then rerun the checkout wizard to recreate the pages. after they are created, move the code block on the checkout page.
