No Progress Bar

Progress Bar

I removed the target="_self" from the form line, removed the progress bar, removed all code associated with WA_DigitalFilePro, removed the folder, including contents, from both local server and remote server, then I rebuilt everything, and still no luck in getting the progress bar to show up. Getting frustrated I stripped the page of all the extra code, detached the template and started over. To my surprise it worked. So I stripped in the code one item at a time testing ever step seeing if any part of the code was causing the problem. Again, I was surprised at the end to still see it working. The only thing I did not do was reapply the template to the page. Does anyone have any ideas why this would happen?
