DELETE FILE not working

you are missing the code that sets the LAYImg hidden element file name  

The first delete is intend to delete a full slide and all the captions in it for that i have this code

all works it deletes correctly the only thing is that warning im getting on the page and when the delete is done it is not redirecting correctly just stays on the same page

i just added the one you say its missing

But still is throwing the warning
