Problems with forms after Data Bridge install in Dreamweaver CC 2014

After installing the Data Bridge extension in Dreamweaver CC 2014 (also tried in the earlier version of CC) I get the attached error message when I first try to insert a form. If I try again the Insert Form command simply doesn't respond. I have removed all Dreamweaver extensions, rebuilt the Configuration folder, and reinstalled just the Data Bridge extension but I still get the same error.

I'm using a Mac running OSX 10.9.3
