to troubleshoot, add the following code at line 1 to turn on error reporting:
<?php error-reporting(E_ALL); ini_set('display_errors','on'); ?>

edit line 764:
$WA_redirectURL = "content1_detail.php?contentID=[Insert_ID]";

$WA_redirectURL = "";

so there is no redirect after the insert.

and just before the closing </body> tag, add the following code that will write the insert statement, the contents of the files array and the file upload debug information to the page:

echo("insert SQL: ".$WA_Sql."<br />");
echo("debug info: ".WA_DFP_AllUploadsDebug()."<br />");

post back any errors and the result of the files, SQL and debug info after trying the upload.
