for the ajax issue, i would need to see the code in context, send a copy of the entire page in a zip archive, not just the ajax code snippet.
for the email body:
When using a template, the labels in the email use the form element names.
to use more user friendly labels in the email, use more user friendly element names.
The email templates don't sound like what you want to be using though where you want more control of the placement of the form data. in this case, unselect the option to use a file to manage the email, and click the DW Edit button to design the body.
this will open a blank dreamweaver page and allow to create the email content using dreamweaver. You can Type your own static label names, then use the bindings panel to add the form elements to the email display.
