Adding Menu Items to the Main Menu Bar


Is there a good step by step thread/tutorial on how to add more items to the main menu bar?
I have altered the menu.php to show this menu:

<!-- Another custom main menu -->
<li class="parent top">
<a href="../../../page.php?Custom Menu=1">Test Menu</a>
<ul class="level-1">
<li><a href="../../../contactus.php">Entry 1</a></li>
<li><a href="../../../contactus.php">Entry 2</a></li>
<li><a href="../../../contactus.php">Entry 3</a></li>

and I created a new php page and added the code per the instructions under "development notes".
However, when I click on the link in the menu it opens in a blank page with no formatting.
This seems to be a little more complicated than necessary. Or am I just missing something basic?
