you will need to create the PDF file and upload it to your server.
Create the form page and add the Create Email Message behavior to send the email, If you select to use one of the templates for the email body, the form data will automatically be added to the email. On the attachments tab, add the PDF as an attachment.
Add the Data Assist insert record behavior to store the form data to your database. On the Server behaviors panel, click the plus button, select webassist -> Data Assist -> Insert Record -> Insert Single record.
on the bindings tab, select each column, click the lightning bolt next to value and select the corresponding form field.
