It looks like you have started another thread on the formmail.pl subject. This is not a script that I have used myself but it seems that it is used by having your PHP page with a form in it post to this formmail.pl.

Since this is not a WebAssist tool or product you may not see much of a response from the engineers here but maybe someone else in the community has experience with this.
