checkout wizard

I'm trying to map a new connection in checkout wizard and am having problems, the questions asked are connection name
MySQL server
Username
Password
database

Pretty sure I got the username and password but confused about the others. My assumption is that the connection name is just what I am calling it, so that leaves MYSQL server and database.

This is the information I have
database name and username
Admin URL (phpMyAdmin)
Hostname
Host IP

Which one is the MySQL server?
I am starting to beat my head against the wall; don't even know what to ask? I do get an error of MySQL error #:2002 if that means anything.
