The first rule when working with forms:
All form processing must occur on the forms action page.

in both forms you sent, the action of the form was to post to another page. this means that the Insert behavior and the email behavior need to be added to the action page. you have added the insert behavior to the same page as the form.

The easiest way to do this is to set the action of the form to post to itself, then add the email and insert behaviors to the same page as the form, in the email behavior, set the Go To option to the page you would like to load after the record is inserted and the email sent.
