Server error accessing "Add/Manage Simple Options"

I've upgraded to Powerstore 4.5 but I'm having a problem when trying to access "Add/Manage Simple Options" - I'm getting the following error page:

Server error
The website encountered an error while retrieving options_add_simple.php?ProductID=5. It may be down for maintenance or configured incorrectly.
Here are some suggestions:
Reload this webpage later.
HTTP Error 500 (Internal Server Error): An unexpected condition was encountered while the server was attempting to fulfill the request.

I can access Add/Manage Advanced Options no problem.

I've searched the Forum and have run the SQL script "settings_synch.sql" but it hasn't made any difference.
