I did not touch the form button name on the checkout page.
And yes, it's going to checkout_failure. This is the page it's going to.. (didn't even notice till now, but it looks like it thinks it's a shipping failure??)

And how do I open a support ticket since you guys don't let us do that anymore (unfortunately).
It looks like the code should not run until the button "Submit_order" is pressed.
Is it possible you updated the button on the confirm or checkout page? You may need a support incident to debug this issue since it looks correct to me. I must be missing something.
