Hey guys, just got back from a long weekend and called Adobe. They said it is the responsibility of the extension producers to make sure their extensions are compatible with Adobe's software. The 12.1 update changes the way Dreamweaver handles Forms, which to my non-programmer mind tells me that this folder was not deleted by accident. They are not admitting any issues at this point. Thanks Nico for the fix and thanks Jason for helping to try to figure this out.

Unfortunately, with Creative Cloud, Adobe is experimenting with a new distribution system, and we could end up with multiple versions of Dreamweaver. So, having CS6 is not good enough, we will have CS6 regular, and CS6 Creative Cloud, and they may be different.
