Labels not showing in Security Assist user update form

I have a User Update form that does not show any labels. The code is as follows for the first label in the group of 6 fields:

<div class="lineGroup">
<label for="User_Update_group_User_Name" class="sublabel" > User Name:<span class="requiredIndicator">&nbsp;*</span></label>
<div class="errorGroup"> <span id="User_Update_group_User_Name_Spry">
<input id="User_Update_group_User_Name" name="User_Update_group_User_Name" type="text" value="<?php echo $row_SecurityAssistusrs["user_name"]; ?>" class="formTextfield_Medium" tabindex="1" onBlur="hideServerError('User_Update_group_User_Name_ServerError');">
<?php
if (ValidatedField('userupdate','userupdate')) {
if ((strpos((",".ValidatedField("userupdate","userupdate").","), "," . "1" . ",") !== false || "1" == "")) {
if (!(false)) {
?>
<span class="serverInvalidState" id="User_Update_group_User_Name_ServerError">Please enter a value.</span>
<?php //WAFV_Conditional userupdate.php userupdate(1:)
}
}
}?>
<span class="textfieldMinCharsMsg">Minimum number of characters not met.</span><span class="textfieldMaxCharsMsg">Exceeded maximum number of characters.</span><span class="textfieldRequiredMsg">Please enter a value.</span></span></div>
</div>

And the CSS is as follows:
form.Basic_Default label.sublabel {
font-weight:normal;
text-align:right;
width:120px;
float:left;
display:block;
margin-top:5px;
margin-bottom:5px;
padding-right:5px;
color: #000;
}

This form is created by the security assist wizard, and the only changes that I have made is to reduce the length of the built in error messages so that they don't spread across a 22 inch screen when displayed. The text should show up and I cannot figure out why it does not. Unfortunately the code is live on line, so I cannot post a link to the page, and it has 16 associated files, and is protected.
Suggestions as to how I solve this problem would be welcome.
