Adding fields to checkout

I have 2 fields that I need to add to my checkout page, I have added a select dropdown, and a text field. How do I get them to record to the orders table on submit? I have added columns for these in the database table already.

I also have an issue with needing additional product information in the cart as well. I need to be have my product sku in the order detail table, but I am only getting productID. My clients do not need the SKU so i did not display it in my product detail page.

Lastly, Ray helped me create a check box in my add to cart, but that is not recording in the order detail either.

Any help getting these records in my database is greatly appreciated!!
