shipping not calculated on some orders

I'm getting some orders in which shipping is not being calculated. It happens at random. The item definitely has a weight on the related pages. I've checked, plus, of course, most orders have the shipping added. So I can't figure out why some orders are getting through without shipping charges.

Any suggestions?
