Checkbox to add second item to cart

I can't seem to find an example that fits my needs. I have several thousand widgets in my catalog that require 1 of 10 possible battery combinations. Some widgets include batteries and some don't, so I use scripting to work out when/which battery set is needed. Also, I'm using my own catalog database.
I need help making a checkbox in my ATC form that will add the selected battery part by ID to the cart along with the widget and in equal quantities as the widget. i.e. 2 widgets and 2 sets of AA batteries with 1 click.
