1) The Hidde nForm element is named "UserDefaultPhoto", however in the Digital File Pro Server Behavior, you have the default file bound to "UserPhotoDefault".

You will need to bind the Default file to the "UserDefaultPhoto" form element.

2) In the Update Record server behavior, the UserPhoto column is not bound correctly. Select the UserPhoto column and click the Lightning Bolt next to value. Expand the WA Upload Files (UserPhoto) Binding Group and select the Server File Variable.
