ok, 2 problems:

1) code order. The code that sends the email is at lines 5 - 93. the code that creates the recordset is at line 114 - 155. the email code block needs to be moved so it is after the recordset code block.

2) Like i said initially, the forms action needs to pass any quesrystring variables on post so that the recordset can be popularted when the email sends.

change the form tag:

<form class="NewFromBlank_Default" id="DHQSimpleContactForm_NewFromBlank_Default" name="DHQSimpleContactForm_NewFromBlank_Default" method="post"  action="<?php echo(htmlentities($_SERVER["PHP_SELF"], ENT_QUOTES)); ?>">

<form class="NewFromBlank_Default" id="DHQSimpleContactForm_NewFromBlank_Default" name="DHQSimpleContactForm_NewFromBlank_Default" method="post"  action="<?php echo htmlspecialchars($_SERVER["PHP_SELF"]); ?><?php echo (isset($_SERVER["QUERY_STRING"]) && $_SERVER["QUERY_STRING"] != "")?"?".htmlspecialchars($_SERVER["QUERY_STRING"]):""?>">
