File uploads in CMS fail

I am having problems with uploading/inserting images with the cms of a Design Extender site. When browsing for a file to upload, the page returns a series of messages from the webpage giving details of filename/size/type etc. The upload fails and although you can return to the content edit window the system has logged the user out and no more edits can be made without re-logging in. Despite refreshing all the online files this error persists. I cannot replicate this behaviour on my testing server - all seems OK.

I am able to ftp files into the created "files" folder (and created sub-directories ) and can use these without error.
