JS Errors when using Form Fields in logging

have tried this another few times, have now 25 block mails in my email templates folder, so u have an idea..........
Same result every time I add the insert behaviour.
have removed the hidden fields, same result, thought maybe this the problem.
Changes to insert on success and then both success and failure, same result.
Each time it spits me the javascript error and if i try to edit the form after creation it sticks a new form in the header and again spits the error.

Tried also creating the form and adding email functions with universal email seperatly, and again spits the same error.
And yes before you ask :) have deleted the cache file every time time, last thing now is reinstall the plugin. Guess will let u know how that one goes.

Am totaly lost now, nothing else to try.
