Error when updating MySQL entry

Hi,

I have created a form to be able to update data from the MySQL database. When I submit the form, I got these error messages:

Notice: Undefined offset: 10 in /clientdata/zeus-dynamic-1/e/c/ecloyalty.biz/www/WA_DataAssist/WA_AppBuilder_PHP.php on line 134

Notice: Undefined offset: 1 in /clientdata/zeus-dynamic-1/e/c/ecloyalty.biz/www/WA_DataAssist/WA_AppBuilder_PHP.php on line 136

Notice: Undefined offset: 1 in /clientdata/zeus-dynamic-1/e/c/ecloyalty.biz/www/WA_DataAssist/WA_AppBuilder_PHP.php on line 136

Notice: Undefined offset: 2 in /clientdata/zeus-dynamic-1/e/c/ecloyalty.biz/www/WA_DataAssist/WA_AppBuilder_PHP.php on line 137

Notice: Undefined offset: 2 in /clientdata/zeus-dynamic-1/e/c/ecloyalty.biz/www/WA_DataAssist/WA_AppBuilder_PHP.php on line 137
Cannot add or update a child row: a foreign key constraint fails (`ecloyalty_ecloyalty_biz`.`tblproject`, CONSTRAINT `fk_uname2` FOREIGN KEY (`uname`) REFERENCES `tbluser` (`uname`) ON UPDATE CASCADE)

Please help me to look at this. I have no idea on how to fix this issue. I have attached the files as well.

Thank you!
