the blank lines and comments betwwen php code blocks areound lines 61 - 79 are causeing the error.

php:

<!-- UPLOAD FILE LINK COMPONENT - 1st upload - (also add session variable to email results template) -->
<?php
if (!session_id()) session_start();

?>
<!-- /end UPLOAD FILE LINK COMPONENT -->

<?php require_once("../webassist/email/mail_php.php"); ?>

you can have any output (even a blank space) between the code blocks in the head.

see the following post for details:
showthread.php?t=19285

change that code to:

php:
?>

<?php
//UPLOAD FILE LINK COMPONENT - 1st upload - (also add session variable to email results template)
if (!session_id()) session_start();
if(
$_SERVER["REQUEST_METHOD"] == "POST")     {

$_SESSION["updFile"] = $WA_DFP_UploadStatus["WA_UploadResult1"]["serverFileName"];

$_SESSION["updFile2"] = $WA_DFP_UploadStatus["WA_UploadResult2"]["serverFileName"];

//end UPLOAD FILE LINK COMPONENT
?>
<?php
require_once("../webassist/email/mail_php.php"); ?>
