hard to tell whats wrong without seeing the code in the WA_Universal_Email/WAUE_test_1.php file.
Try this, though:
on the Server Behaviors panel, double click the Universal Email server behavior to edit the email.
On the body tab, click the DW Edit icon. in the Dreamweaver page that comes up, go to the Bindings panel, expand the form collection. select the contact_name dynamic data binding and drag it to the page where you wish it to display. The save the file to get back to Universal Email and click OK.
Upload the entire WA_UniversalEmail folder to the server and test again.
