well, they way your add to cart buttons are set up, the products are not the same product with options. they are set up as 11 separate products with 11 separate product ID's.

create another column in the cart, lets call it "shipGroup"

for each product that should be in the 6.95 shipGroup, set the value to 1 in the add to cart button, for each item that should be in the 11.95 shipGroup, set the value to 2 in the add to cart behavior.

then configure a rule for each shipGroup:
"number of items with a specific column value"
if the number of items where "shipGroup" has a value of "1" is greater than "0"
"Flat Rate"
(ceil($this->ConditionalTotal("Quantity", "shipGroup", "1") / 5)) * $this->ConditionalTotal("Quantity", "shipGroup", "1")

the rule for shipGroup 2 will be very similar, just sub 2 instead of 1
