User Email Form Question

1) All I would need to get from my users in the text field is their comments. Their email address is already shown by gmail. Also, the button name is, for my purposes, unncecessary.

2) I'm going to eliminate the name and the email address input fields (in the "send" form) and replace them with dynamically entered email and name, so those two elements are also unnecessary for my purposes

I just tried eliminating some of those elements, but the "Dynamic value" pre-existing in the template already contains most of them. Any way around this? (I hope this is clear enough for you)

So, basically, the fields I would like to receive in the email body (text) are just the comments and perhaps the first name. Is this possible?
