file not found checkout_success.php

I have a client who is till on PS3 and they are starting to get this error or This webpage has a redirect loop
The webpage at checkout_success.php has resulted in too many redirects. Clearing your cookies for this site or allowing third-party cookies may fix the problem. If not, it is possibly a server configuration issue and not a problem with your computer.
Here are some suggestions:

Reload this webpage later.
Learn more about this problem.

Error 310 (net::ERR_TOO_MANY_REDIRECTS): There were too many redirects.

The transaction complete's properly. In some cases the customer has been charged three times.

I've tried it and was only charged once, however the cart wasn't cleared and I received the file not found error for checkout_success.php even though the file is present on the server.

The email are generated correctly.

Any clues where I should look first?
