for the price, i think you will want to have 3 price columns

Price
Price250ml
Price400ml

Use the Price Column as the base price, and the other 2 as Incremental prices. for example in item EXT300, you would set the values in the add to cart button as:

Price: 13.00
Price250ml: 16.00
Price400ml: 37.00

Item EXT308 would be set as:
Price: 15.00
Price250ml: 18.00
Price400ml: 40.00

etc.

then on the calcualtions tab, create a new calculation called truePrice as:

[Price] + ((abs([Size] == '250ml'))?[Price250ml]:0) + ((abs([Size] == '400ml'))?[Price400ml]:0)

then update the TotalPrice calculation to:

([Price] + ((abs([Size] == '250ml'))?[Price250ml]:0) + ((abs([Size] == '400ml'))?[Price400ml]:0) ) * [Quantity]

in the cart display, you will need to change the edit the code that display the price:

php:
<?php echo WA_eCart_DisplayMoney($yourCartName$yourCartName->DisplayInfo("Price")); ?>

to display the truePrice instead:

php:
<?php echo WA_eCart_DisplayMoney($yourCartName$yourCartName->DisplayInfo("truePrice")); ?>

[/php]

  When making product pages, do I layout the image, product description and anything else about the product on the page first? And then place the add to cart button beside it?

yes, add the add to cart button to the page after you have configured the product layout.
