File is not attached in email

Ok Ray, here's my update. After my last issue, I made a simple test form page using some of the coding you've used in the past. I added a text field to that form, initiated UE and uploaded the form.

Here's what I'm getting when I go to that page:

Fatal error: require_once() [function.require]: Failed opening required '../webassist/email/mail_php.php' (include_path='.:/usr/local/php5/lib/php') in /home/content/99/7980399/html/upload_rev/test.php on line 1

Re test.php: If I remove these 1 and 2, the form shows up, but when I submit, I get an error. When I put those two lines back in, I get first error above.

Could the problem lie in the UE coding?
