File Upload Problem. Has Worked Before. What am I Missing?

Thanks Jason. That was the problem. I don't recall ever noticing that before. As you can see, I strip out the data assist formatting for my own, but not sure how that got eliminated at some point.

Works great now, thanks for your help!

By the way, while we're at it, how do I make a feature request for the next update of data assist? It would be really nice to have a non-formatted option that would just generate the code only for each of the pages, without any table and css formatting, for those of us who want to highly customize the layout. It would be a real time saver not to have to manually strip everything out for each iteration.
