Image received but no Text!

I just using Universal Email for the first time. Have created a form in Dreamweaver CS5 and have applied Universal Email. Loading both the form page and the webassist folder to the remote server and testing via the site I received an email with the image attachment but none of the text!??!
I think I've followed your Getting Started Guide but checking the various pages of the "site" I can't see any mention of the text part of the email, should I?
I've attached the form page and webassist folder for you.
Thanks for your help in this matter - it must be simple but perhaps I'm too simple!
