Map/Directions Do Not Show When Sending Form Data To It

I have a page the displays a Pro Map with the hard coded destination address entered at design-time and it works fine. On that same page I also have an address form that once submitted opens a new page with another pro map to show driving directions from the form address.

I've tested that the form is successfully submitting the data but I just get a blank map placeholder and nothing else. Below is the page with the form for testing.

I am using Dreamweaver CS4 and the 1.0.4 version of the extension.

It seems to work for me now. You must have found the issue. If not please give me more information to allow me to reproduce the problem and I can look into it further.
