What's up with the UL and LI tags in my form?

I had to create my form again and I'm getting some weird formatting including bullet points on my form.

Issues like this with the formatting can result if you do not ensure that all of the css is properly uploaded. Do you have a link to the live page this can be seen at? If I could take a look I would have a better idea of what is going on with the page.
