Surveyor fails when selecting design style for html sitemap

Hello!
I am having a problem with Surveyor when creating an html site map, the problem occurs when selecting design style for the site map. (dreamweaver stops responding)

Dreamweaver Version: CS5
Surveyor Version: 1.0.8

Note:
I installed and uninstalled surveyor 4 times
I deleted dreamweaver preferences
I tried it with other sites with no success
(the extension creates xml site maps with no problem, the issue is only with html site maps)
Also I tried to create the html site map using both methods (with the xml file and select pages manually, both failed).

Any assistance will be appreciated
