Per the frequently asked questions:

Can Surveyor create multiple-column HTML sitemaps?
Yes. Surveyor gives you the ability to add column breaks to change the layout of your HTML sitemap page.

I can't find any way to change the layout other than the order of the links. Please point me in the right direction.

On the configure Site Map settings page, select the last page to be in the column, then click the plus button and select Column Break.
