CS5 Surveyor JavaScript error

Creating a HTML sitemap creates an error like shown in screenshot:
Put it manually, writing missing parameters by hand.
After submitting I get the following messages:
* Yahoo: Sorry, Forbidden.
You don't have permission to access this URL on this server.
* Google:
Bad Request
Error 400
*Ask.com:
Your Sitemap submission was successful
Your Sitemap has been successfully received and added to our Sitemap queue.
Thank you for your submission!
*bing:
Thanks for submitting your sitemap.
*api moreover:
Thank you for your ping. The moreover database will be updated with the new data from your url -

Can you send me a link to the sitemap that you're submitting? And also attach your xml site map so we can test the html sitemap generation?

I had to get going and wrote everything by hand. This was faster than debugging this extension.
